Medical Cannabis Certification

Medical cannabis -- or marijuana -- is a popular treatment option for patients suffering from chronic pain.

Patients who qualify may be eligible to receive a Medical Cannabis Certification Form if they are on an interventional care plan. This form is required by the state of Missouri for patients with eligible medical conditions.  Receiving an Identification Card from the Missouri Department of Health and Senior Services allows them legally purchase, possess, and use cannabis.  There is a $30.00 office fee for completing this paperwork.

To be qualified patients must be established with this office (a patient for three months or longer) and also be undergoing current interventional therapy

Our office does not see cannabis-only patients.

Patients seeking medical cannabis will follow and complete the process laid out for them by Missouri DHSS. They will first obtain a certification form from us and then complete the state's process to be approved for an Identification Card.
